Companionship and Connection Are Just a Visit or Phone Call Away

Promoting Independence and Helping Older Adults Stay Safe and Live Well Wherever They May Be.

Loneliness and social isolation have become an epidemic among older Americas. The evidence overwhelmingly supports that many adults ages 60 and older are lonely or secluded in ways that pose a severe threat to their health or wellness.

We recognize how important it is for older adults to maintain their independence while feeling remembered, safe, and able to enjoy one-on-one connections. We are more committed than ever to providing the care and companionship needed to build a culture of togetherness by supporting older adults through the aging journey.

The Comfort of a Friendly Voice

Designed to encourage socialization, Friendly Visiting and Telephone Reassurance – Solano is a FREE program that helps vulnerable older adults build friendships and connect with resources they may not have been able to access otherwise.

We offer friendly visiting through weekly telephone calls, video conferencing, in-home visits (when safe*), social groups, pen pals, friendly pet visits, and a variety of other ways to engage with clients to ensure health, well-being and a sense of community.

Our trained team of staff and volunteers also assesses for safety risks and any unmet needs. Our team can connect older individuals with resources such as nutrition assistance, medication reconciliation, fall risk management, and so much more.

We aim to empower individuals ages 60+ who may be homebound, bedbound, isolated or at risk for loneliness and social isolation. While this program is community-focused, we are also proud to provide services to older adults in facilities.

*Since the coronavirus pandemic began, it has changed the world, our community, and the elder care industry as we know it. Given the immense impact on older adults, our team will be adhering to all COVID-19 regulations while maintaining quality interaction and care throughout the pandemic.
