You have the right to file a grievance regarding a complaint or an incident, and to submit feedback of any services provided to you.

All grievances are confidential and rights are protected.


A grievance may be submitted verbally or in writing to Empowered Aging’s senior leadership staff. Call 925-685-2070 to report verbally and/or complete the grievance form listed below to report in written form. Empowered Aging’s leadership staff will respond verbally or in writing within 7 working days of receipt of your grievance.
